A digital version of the Advisor Feedback Form is online right here:

In order to use the form, someone needs to:

1) Load that link
2) Fill it out in their web browser
3) Hit submit

All of the submitted data will go into a private spreadsheet. Individual advisory feedback will then be emailed to each advisor that uses the form.

Note:
- The form can be filled out from any internet connected computer (or mobile device), without logging in to any systems. So, a student could do it at home, for example.
- The data submissions are totally anonymous. The only data that is collected beyond what the person puts in the form is the time stamp of the when the data was submitted.
